Accepted Insurance

We are providers for most of northwest Ohio's major health insurance plans. If you have medical insurance, we are anxious to help you receive your maximum allowable benefits. Prior to your first visit, please contact your insurance company to verify that our practice is an accepted provider in your plan.

 

Once you have confirmed your insurance plan's acceptance, we will be happy to process your insurance claims for you.

 

In order for us to submit your claims, you must provide the following at all visits:

 

1) Copy of your current insurance card

2) Photo ID that matches the last name as the name on your insurance card

 

Your Payment Responsibilities

 

Most insurance requires and office visit co-pay. This payment is due at the time of your appointment, unless payment arrangements have been approved in advance by our staff. We accept cash, checks and Master Card or Visa.

 

For services beyond an annual office visit, it is your responsibility to obtain preauthorization from your insurance company. It's always a good idea to contact your insurance company and check for coverage prior to any appointment beyond annual wellness exams.

 

Once a claim has been processed by your insurance company, it is your responsibility to pay any outstanding amount not covered by insurance.
